How do I request my OSU transcript?

All OSU transcript requests must be in writing with the student's signature. You may request your transcript in person, by mail, by fax or (in some cases) on the Web.

In order to facilitate locating the correct record, please include as much of the following information as possible when requesting your transcript.

  • Full Name (Your name as it is now and as it was when you were enrolled at OSU, if different)
  • Dates of attendance
  • Student ID or CWID or Social Security Number
  • Birthdate
  • Full address to send the transcripts, along with your email address

Be sure you sign and date your request (SIGNATURE is required because of the Federal Educational Rights and Privacy Act).

OSU-Tulsa is able to print transcripts for students who were enrolled from Fall 1999 forward. Any transcript requests for students prior to Fall 1999 will be faxed to the Stillwater campus for processing.

Requests via fax

You may fax your request: (be sure to sign your fax) to 918-594-8307. If faxing the request, be sure to enclose a telephone number where you may be contacted in case there are any questions regarding your request or the request is not legible. The Enrollment Services office telephone number is 918-594-8020.

Because transcripts are printed on Scrip-Safe paper which does not photocopy or transmit legibly, we do not fax transcripts.

Express Delivery

For express delivery outside the United States, a prepaid mailer (either FedEx or DHL) must be provided to the Transcript Office. This can be done in person, by mailing in the prepaid mailer or by having a third party bring the prepaid mailer to the Transcript Office.

Third-Party Pickup

You may give a third party written permission to pick up your transcripts and send them for you. Authorize a third party to pick up transcripts by faxing your signed authorization directly to OSU-Tulsa Enrollment Services at 918-594-8307. In your request you must state the name of the person who has permission to pick up your transcript and how many copies may be picked up. After the signature has been confirmed, the third party may pick up the transcript(s) by showing a valid ID.
