Tues., Oct. 20 | 2:30-4:30 p.m. | NH Lobby
Open to all OSU-Tulsa students, faculty and staff. Flu shots are FREE for currently enrolled OSU-Tulsa students.
Faculty and staff must bring their insurance cards. Cost of flu shots for faculty and staff will be covered by most insurance.
OSU ID required for all participants.
Consent for Vaccination Form
Fill out your Consent for Vaccination Form ahead of time and bring it with you to save time waiting for your flu shot. All participants must fill out sections A, B, and C. Employees will need to fill out Section D and bring their insurance cards.
