  1. What is the Evaluation period?
    A: April 2018 to April 2019
  2. When can I launch my review?
    A: Reviews are available from Jan. 2 to April 15, 2019.
  3. Which review do I need to select?
    A: There are 3 forms. Select best fit.
    1. CHS/Tulsa Staff (staff with no supervisory role)
    2. CHS/Tulsa Supervisor (staff with supervisory role)
    3. Director and Above (staff with Director title or above)
  4. How does my supervisor know I have completed my portion?
    A: An email is automatically generated when the staff member completes and submits their portion.
  5. To whom does the performance review form go to and when?
    A: The form is started/launched with Self Review by the employee.
    • Employee’s self-review > Employee’s supervisor/manager (input, rate & one-on-one meeting with employee) > Employee for signature > Indirect supervisor/manager for signature > HR
  6. Can I save my work and come back to finish later?
    A: Yes, you can also cut and paste from a word document.
    1. You can add color, italicize or bold your text
    2. Number or bullet point your text
    3. You can even save/print your review to remember what you wrote for the one-on-one meeting
  7. Can I attach documents?
    A: Yes, up to 3 documents can be attached.
  8. Will I be able to see my review in the future?
    A: Yes. You can view the review any time after the manager submits the form. You will have to select “Completed” in the drop down menu to view completed performance reviews (contact HR to add).
  9. Will I be able to submit an appeal or rebuttal to my review?
    A: Yes. You can submit within 10 days of the completion of your review and it will be attached to that review.
  10. What happens if my supervisor or indirect manager is not available i.e. out of state, left the university, roles change?
    A: HR can add another person to the workflow (contact HR for any workflow issues)
